• Age 0-21 for the Diagnostic Program, 0-23 for the Treatment Program

• Under the care of CMH-approved doctor (M.D or D.O.) and who have or may have a chronic medical condition

The Children with Medical Handicaps Program (CMH) is a health care program in the Ohio Department of Health (ODH). CMH links families of children with special health care needs to a network of quality providers and helps families obtain payment for the services their children need.

Services Provided:

• Funding services for the diagnosis and treatment of medically eligible conditions.
• Collaborating with public health nurses and local health departments to assist in increasing access to care.
• Supporting service coordination for children with selected diagnoses.
• Assisting families to access and utilize appropriate sources of payment for services for their child.

Although CMH covers a wide range of services, it is important to know CMH does not cover all the services a child with special health care needs may require, nor are all services available for every diagnosis. Services must be related to the child’s CMH-eligible condition.

The purpose of the Save Our Sight Program is to ensure that children in Ohio have good vision and healthy eyes. Eligibility is based on the various programs Save Our Sight includes.

Services Provided:

• Parents and caregivers can apply for free protective eye-wear for youth sports and school activities and preventing eye injuries.
• If diagnosed with Amblyopia (lazy eye) they can get support and free patches from the Ohio Amblyope Registry.
• Professionals that work with children can request free eye health and safety programs for their schools or community organizations.
• Vision screening of children by providing free training and equipment to screeners.

The Ohio Hearing Aid Assistance Program (OHAAP) provides assistance to families with children, birth to twenty-one years of age, with hearing impairments, to purchase hearing aids, assistive listening devices, earmolds, external cochlear implant processor replacements, and/or hearing aid batteries.

Families with incomes at or below 400 percent of the federal poverty guidelines are eligible for the program.  Children enrolled or who can qualify for Medicaid or the Children with Medical Handicaps Program (BCMH) are not eligible for OHAAP (unless documentation can be provided that Medicaid of CMH will not pay).  

A family’s eligibility can be determined using the federal poverty guideline table. Depending on income limits, families may be responsible to pay a sliding scale fee up to $20.00. The fee is for the audiological services to assess the child for the hearing aid/assistive listening devices and external cochlear processor replacement, as well as needed follow up to evaluate aid. Families who qualify and are interested in the program must complete the Family Application Form and provide verification of income.

WIC is the Special Supplemental Nutrition Program for Women, Infants, and Children. WIC helps eligible pregnant and breastfeeding women, women who recently had a baby, infants, and children up to 5 years of age.

WIC provides nutrition education, breastfeeding education and support; supplemental, highly nutritious foods such as cereal, eggs, milk, whole grain foods, fruits and vegetables, and iron-fortified infant formula; and referral to prenatal and pediatric health care and other maternal and child health and human service programs.

Grants wishes to children with life-threatening illnesses and assists those experiencing poverty, abuse, or homelessness. Offers funeral cost assistance to families who have lost a child. 

Completed applications must be returned to the Trust by the end of each quarter: March 31st, June 30th, September 30th, and December 31st. 

Provides financial assistance to people who suffer deafness or hearing impairment.  Assistance includes obtaining mechanical devices, medical or specialized treatment or specialized education as well as speech classes, note takers, interpreters, etc. and in other areas of need that are directly related to hearing impairment.

Child under the age of 18 years at time of placement with their service dog. Must have a physical disability, psychological disability, or on the autism spectrum; be enrolled in a qualified training program prior to applying and complete an application.

Provides service dog scholarships for children with special health care needs. 

 Provides assistance to single working parents who: Have a child/children between the ages of 5 and 15; Work at least 20 hours a week; And meet the salary requirements, as detailed in the Application Guidelines.

Provides financial assistance to single working parents who wish to enroll their children in extracurricular and enrichment programs.

Individuals with autism who are non-verbal or minimally verbal and whose communication challenges put them at increased risk of injury or harm. For those in extreme financial need.

Parent-led non-profit organization that responds to the most urgent needs of the autism community, providing real help and hope so that all affected can reach their full potential. Provides expense assistance for assistive technology, as well as information on disease and disability.

Focuses on aid that will benefit children, older adults, and vulnerable populations. Limited to US residents and max of $200 per request.

Provides financial assistance wish granting to applicants who successfully demonstrate need.

Low income individuals in need who are referred by professionals.

Assists low income individuals with a range of needs including mobility equipment (wheelchairs, walkers, vehicle modifications such as hand controls and lifts), ramps and minor house modifications for accessibility, medical equipment (not including medical bills or back bills), and prostheses. Applications accepted from referring professionals only (social worker, case manager, teacher, school counselor, minister, etc.).
