New Mexico Projects
New Mexico has the following projects and initiatives that support
children and youth with special health care needs and their families.
- Care Coordination in New Mexico The New Mexico Care Coordination Consortium (NM CCC) supports the work of the NM Title V Children with Special Health Care Needs program to promote access to a comprehensive Medical Home for all children. Coordinated care for children with complex and chronic conditions and their families can help with better outcomes.
- New Mexico is the recent
awardee of a PMHCA grant. The Pediatric Mental Health Care Access (PMHCA)
program is a HRSA grant that promotes behavioral health integration into
pediatric primary care using telehealth. The overarching goal of the program
is to use telehealth modalities to provide timely detection, assessment,
treatment, and referral of children and adolescents with behavioral health
conditions, using evidence-based practices and methods such as web-based
education and training sessions. New Mexico is one of many state awardees in

also offers additional services for those who meet financial and medical eligibility guidelines. CMS is a supporter of the
Medical Home Portal's partnership with New Mexico.
