The Pediatric Patient Summary-Medical Home Portal (PPS-MHP) project will build on the Medical Home Portal, live since 2001, and the Pediatric Patient Summary, initiated in 2010 – projects of the University of Utah's Department of Pediatrics and Department of Biomedical Informatics (BMI).

The Pediatric Patient Summary (PPS) will provide a collaborative online platform where clinicians, care coordinators, and parents/guardians/patients create and maintain a concise, accurate, and up-to-date summary of relevant information about individual children and youth with chronic and/or complex conditions. The PPS will gather information from the electronic health record (EHR) and from other electronic sources through health information exchange (HIE) and provide tools to enable clinical team members to easily curate it. Parents/guardians/patients can view the information, question elements, offer explanation/annotation, and add personal preferences, relevant history, family history, social context, and care/life goals.

For clinicians and their teams, the PPS will exist as a SMART on FHIR “app” that plugs into Epic and other compatible EHRs, presenting as a native window/tab. In Utah, parents/guardians/patients will access the Family PPS through their EHR's Patient Portal on a computer or mobile device. For clinicians who don’t use Epic or a compatible EHR, the summary will be available through Utah's clinical health information exchange (CHIE), and other health information exchanges in the future, and the parent/guardian/patient will be able to share it directly with anyone, such as ED clinicians, new subspecialists, schools, other family members, etc.

Access to information from the Medical Home Portal (MHP) and its directory of service providers will be integrated into the PPS. The Portal's information focuses on the needs of primary care clinical teams – knowledge, guidelines, management recommendations, clinical tools, patient education, and local service providers. The context of a patient visit (diagnoses/problems, age, medications, etc.) will guide what information is offered and users can also easily link out to the MHP site to find more. We will build on the PPS-MHP app to add Care Team Management tools and a platform for Care Planning.

The PPS-MHP project is a 4-year collaboration among Pediatrics, BMI, the Utah Health Information Network (UHIN) (UHIN), Intermountain Healthcare, and the Utah Department of Health & Human Services. It is supported through the Utah Medicaid program’s 2017 State Health Information Technology (HIT) Implementation Advance Planning Document-Update (IAPD-U). Funding was awarded 9/1/17 by the Centers for Medicare and Medicaid Services (CMS) and the Office of the National Coordinator for Health Information Technology (ONC) with funds allocated by the Health Information Technology for Economic and Clinical Health (HITECH) Act of 2009 ( 258 KB), part of the American Recovery and Reinvestment Act of 2009 (ARRA).
